For Critical Access Hospitals (CAHs), the Rural Health Transformation Program (RHTP) may be the most consequential funding opportunity in a generation — and the window to act is narrow. The money is real, the timelines are tight, and the hospitals that engage early will capture far more than those that wait. Here’s where things stand and what your leadership team should do next.
Where the Program Stands
RHTP is a $50 billion federal program established under Public Law 119-21 and administered by CMS, delivering $10 billion per year from FY2026 through FY2030. Half of each year’s funding is distributed equally among approved states; the other half is allocated at CMS’s discretion based on factors like rurality, state rural health metrics, and projected impact.
On December 29, 2025, CMS announced that all 50 states were approved for first-year awards — averaging roughly $200 million per state, within a range of $147 million to $281 million (CMS, Dec. 2025). The program has now pivoted from planning to execution. States submitted finalized spending plans in early 2026, and funds are moving to state-selected subrecipients — including CAHs. In several states that pipeline is already open; Michigan, for example, has named more than 100 subrecipients across health IT, workforce, and network development.
The Wins
The scale is unprecedented. Every state is funded, and rural health has a rare, sizable capital infusion aimed squarely at the things CAHs struggle to finance on their own: workforce, telehealth, technology modernization, cybersecurity, and value-based care models. For hospitals that have deferred investment for years, RHTP is a chance to fund transformation rather than just survival.
The Challenges

Leaders should be clear-eyed about the risks:
- It’s one-time, time-limited money. The program runs five years and is not permanent. The National Rural Health Association has stressed that sustainable, long-term rural funding is still needed beyond FY2030 (NRHA, Dec. 2025).
- The spend timeline is aggressive. First-year funds must be expended by September 30, 2026 — a genuine administrative challenge given how recently disbursement began (CSG, 2026).
- Access runs through the state. CMS sends dollars to states, not directly to hospitals. CAHs must actively compete and align with their state’s awarded initiatives to see any of it.
- Out-year uncertainty. Because half of each year’s pool is discretionary, future funding levels aren’t guaranteed.
The Opportunities
This is where strong leadership separates the winners:
- Engage your state now. Connect with your state RHTP office and advisory councils, and map your projects to your state’s approved initiatives. Early alignment is the single biggest predictor of who gets funded.
- Target high-value categories. Position for subawards in workforce development, telehealth, health IT and data infrastructure, cybersecurity, EMS, and behavioral and maternal health.
- Build capabilities that last. Use the funding to create durable assets — partnerships, consortia, shared services — that outlive the five-year window rather than one-time purchases.
- Move in the next 90 days. With the FY2026 spend deadline looming, states are contracting quickly. Hospitals that arrive with shovel-ready, well-scoped proposals will be first in line.

Turn Funding Into Lasting Results
RHTP dollars will reward hospitals that can plan, staff, and execute — fast. That’s exactly where many CAHs feel stretched thinnest. H&H Leadership Solutions partners with rural and Critical Access Hospitals to translate this opportunity into results: advisory support to shape and align your initiatives, leadership and workforce strategy to deliver on them, and the executive talent to lead execution before the clock runs out.
